5. SHARP Employee Name;
   
6. Effective Date;
   
7. Benefit Plan;
   
8. Deduction Code;
   
9. Deduction Amount;
10. Error Message (If Any);

Department

FREQUENCY:

ING will request the report to be run whenever there are changes. The agency will receive an email notifying them the report is available.
