Job Description:

Job Summary: Social Worker Specialist works with the Interdisciplinary Treatment team to assess new admissions and on an ongoing basis assess response to treatment. Discharge planning begins at day one of hospitalization with the patient’s goal being priority. Additionally social workers collaborate with community providers and the patient’s significant others to locate resources for discharge. Social workers are the primary liaisons to families as well as the community resources, relaying information to the team in written form as well as verbally. Assessments and discharge planning that meet accreditation standards take hours per patient.

Job Description: Collects, evaluates, and documents social history and assessment information, as assigned. Gathers information from all available sources to formulate a social history and psycho-social assessment using approved social work methods and techniques. Completes history within five (5) calendar days of the patient’s admission, in order to provide the treatment team with necessary input for patient assessment and treatment planning. Coordinates placement of patient to ensure that patients are released to the least restrictive environment possible, without endangering safety. Uses resources creatively and interactions are flexible. Completes progress notes in order to document progress made toward placement. Documents in progress notes on assigned patient, their response to treatment, involvement by family and significant others, Community Mental Health Center and community support services involvement. Provides social work input into the inter-disciplinary treatment team in development, implementation and review of treatment plans, so that needs and resources are accurately identified and addressed in treatment. Implements treatment plan through interactions with patients, which may include individual and group counseling, observing recognized social work standards and practices.

 

Minimum qualifications: must be licensed to practice social work in the State of Kansas at the time of hire.

 Preferred Criteria; Prior experience working with individuals who have psychiatric diagnosis, but will train the right person.
