The Setoff Agreement requires an authorizing signature and must be mailed in to complete the agreement process between the municipality and the State of Kansas. We are providing a downloadable copy in Word format for your convenience.

- Delinquent Debts Form

Debts should be submitted by completing the Delinquent Debts Form  and sent as an e-mail attachment to the following e-mail address: kssetoff@da.ks.gov

- Debt Adjustment Form

The Debt Adjustment Form is an online form to be used for debt adjustments, collections or decertifying a debt and can be used for up to 5 debtors per form.
