Due to the increased awareness of identify theft we have revised the following Payroll forms to delete the Social Security Number field. Please use the new version of these forms effective immediately. The forms that have been revised are as follows:

DA-6P Paycheck Stop Payment Request
DA-179 SHaRP Maximum Arrears Payback
DA-180 SHaRP Paycheck Reversal/Adjustment/Supplemental
DA-182 SHaRP Paycheck Reprint Request
DA-184 Authorization for Direct Deposit of Employee Pay
DA-186 Authorization for State of Kansas Additional Withholding Tax Deduction
